This page contains important information about our services and business policies so please read carefully and jot down any questions you may have so these can be discussed at your first session.
SELF-ASSESSMENT FORM: Before your evaluation, it is important that you complete the Self-Assessment Form in as much detail as possible. The information not only will be of use during your assessment, it will also help organize your thinking about your medical/psychiatric history and reason for present visit, and help us better understand you and your needs. If there is a question on the form that you do not wish to answer or cannot answer, leave it blank.
CANCELLATION POLICY: We understand that events arise and appointments cannot always be kept and must be rescheduled. We request, however, that you call 24 hours in advance so that time may be rescheduled for another client. The policy is to charge you for the missed appointment and that fee must be paid before the next scheduled appointment. Obviously there are situations that cannot be anticipated and these may be discussed at the next visit.
DISCHARGE POLICY: It is our policy to discharge you from the practice after 3 missed appointments. If you miss two consecutive appointments, discharge from the practice will be considered. Please make every effort to either keep your appointment or call 24 hours in advance to cancel.
CONTACTING PROVIDER: Office hours are variable depending on the provider. Office staff will be able to assist you during business hours. If necessary, your provider will call you back within 24 hours but they are not always immediately available as they are seeing other patients. After hours and in an emergency situation, you can call the answering service phone number provided on The Center for Integrative Health's phone message and someone will cal you back. If you cannot reach someone at The Center for Integrative Health and it is an emergency, or you feel you cannot wait until your call is returned, you should go to the nearest emergency department.
MEDICATION REFILLS: Refills will only be given if you have been seen within the last 3 months or within the time discussed with your provider. Refills will not be provided the same day you request them. You must provide the office with three days notice in order to obtain a refill. If you require a refill the same day, after hours or on the weekends, you will be charged a $20.00 fee for that service. Medications are also not routinely changed over the phone. In order to get a different medication, a visit to your provider is required.
CONFIDENTIALITY: In general, law protects the confidentiality of all communications between a client and practitioner and information can only be released to others with your written permission. At The Center for Integrative Health, we request that you sign a form allowing infomation to be shared amongst the staff here so that a treatment plan may be developed best suited for each individual.
There are also a number of exceptions to confidentiality. There are some situations in which we are legally required to take action to protect yourself or others from harm. If we believe that a child, elderly person, or disabled person is being abused, we must file with the appropriate state agency. If we believe that a client is threatening bodily harm to another, we are required to take protective actions, which may include notifying the potential victim, notifying the police, or seeking hospitilization for the client or to contact family members or others who can provide protection. These situations rarely occur, but if they should, we will make every effort to fully discuss it with you before taking any action.
